Pathfinder Session – £250

“I’ll help you out of the woods — and onto a clear path to care.”

A full care needs review to understand your loved one’s situation. I’ll personally research care providers with capacity and experience in their specific needs, make the first calls, and provide a printed and digital pack including:
- A tailored provider shortlist (with pros and cons)
- A benefits guide
- Local support services
- Questions to ask during care assessments

This is the required starting point before accessing any other services.

Guided Care Setup – £500

“You don’t have to figure it all out alone — I’ll walk the path with you.”

Includes the full Pathfinder Session above, plus:
- Attendance at up to 3 provider consultations or care assessments
- Support reviewing quotes, contracts, and next steps
- Guidance through decision-making and care setup
You’ll have me by your side until the right care is in place.

Hospital-to-Home Help – £500

“When time is short, I’ll help you plan fast — and plan well.”

A fast, remote package for hospital discharge situations:
- I’ll speak with the ward and your family
- Create a care plan based on needs
- Provide suitable provider options and follow-up once home
Designed to reduce pressure and avoid rushed decisions.

This service also fulfils the requirement to access ongoing support.

Peace of Mind Plan – £50/month

“A trusted point of contact — so someone’s always keeping an eye.”

- I’ll act as named contact for emergency alerts/lifeline alarms
- Includes one emergency in-person call out
- Available for light-touch check-ins and reassurance

Only available after completing one of the initial packages above.

Full Circle Support – £150/month

“Ongoing support for care and medication — handled, checked, and in hand.”

- Management of repeat prescriptions
- Liaison with GPs, pharmacies, and care providers
- Regular check-ins with family

Only available after Pathfinder, Guided Setup or Hospital-to-Home package.

Top-Up Time – £50/hour

“Flexible support, just when you need it.”

Extra support beyond your plan. Use this for:
- Extra visits or meetings
- Care reviews
- Admin or liaison time

Available to existing clients only.
